Community resource referral platforms have been growing in popularity as a way for healthcare and social sector organizations to coordinate services for individuals with complex needs — a demand that has intensified during the COVID-19 pandemic. Promising practices on how to structure and maintain these tools and workflows are emerging, but organizations seeking to implement these practices must navigate their unique local environment.
This session, co-hosted with Health Leads, presented what we know about effective referral platforms and discussed what it looks like to activate these recommendations. The conversation anchored in the experience of two organizations: the Camden Coalition of Healthcare Providers in New Jersey and Health Leads’ engagement with Benetech’s Service Net pilot in California. Participants were encouraged to contribute to the conversation and make connections with one another, sharing their own experiences and strategies.
