The Blueprint for Complex Care is a guide for advancing the field of complex care. Incorporating far-reaching perspectives from published literature, interviews, surveys, and convenings, the Blueprint presents a comprehensive picture of the field of complex care and a set of recommendations for building on existing work. Developed by National Center for Complex Health and Social Needs, the Center for Healthcare Strategies, and the Institute for Healthcare Improvement with the support of The Commonwealth Fund, the Robert Wood Johnson Foundation, and The SCAN Foundation, the Blueprint will be released at Putting Care at the Center 2018. In this session, panelists will discuss the Blueprint’s genesis and some of its recommendations for the field.
