Stable housing is a key prerequisite for health, and increasing numbers of hospitals, health systems, ACOs, MCOs, nonprofit healthcare organizations, and more are partnering with supportive housing providers to direct housing resources towards their most vulnerable, highest cost consumers.

This report highlights successful healthcare and housing partnerships to show how the healthcare sector can best invest in housing for their consumers with the most complex health and social needs.
