This evaluation of the Camden Coalition of Healthcare Providers’ 7-Day Pledge program, published in JAMA Network Open, shows that rapid connection to primary care after hospital discharge can help avoid readmission. Patients who had primary care appointments within seven days of hospital discharge had significantly lower numbers of 30- and 90-day hospital readmissions.
