MercyOne, a statewide health care system with the largest primary care network in Iowa, is involved in a significant initiative to assess patients’ social needs and provide assistance in meeting those needs, including embedding community health workers in primary care clinics.
Issue: Social isolation
Social isolation has become the most frequently identified need by patients on the social needs screening. When the need is identified, MercyOne colleagues and the greater community struggle to know the most effective way to help the patients.
Convening:
MercyOne PHSO and the National Center brought together cross-sector organizations, guided by patient feedback, to address loneliness and social isolation in Central Iowa.
Outcome:
The convening focused on setting a shared language, reviewing research and data on social isolation, and catalyzing ideas to better serve patients facing social isolation in the community.
