Defining the field of complex care

The Blueprint for Complex Care is a joint project of the National Center for Complex Health and Social Needs, the Center for Health Care Strategies (CHCS), and the Institute for Healthcare Improvement (IHI). We are working to define the emerging field of complex care and to create a shared set of priorities for all organizations that will contribute to its development.

We’re gathering input from a diverse set of stakeholders from across the country to outline the goals, needs, and priorities of the complex care movement. The finished product will be a strategic plan for the field and a roadmap for the future of innovation, policy, research, and education in complex care.

The Blueprint launched with a Beehive activity at Putting Care at the Center 2017, and will be completed by fall 2018.

This initiative is possible through support from the Robert Wood Johnson Foundation, The Commonwealth Fund, and The SCAN Foundation.
