Oakland, January 10, 2019 – Kaiser Permanente’s Care Management Institute will host a webinar with the National Center for Complex Health and Social Needs titled “Innovative Approaches to Complex Care.” The hour-long webinar will take place January 16th and will focus on innovative best practices used in caring for individuals facing intersecting medical, behavioral and social challenges.

WHAT:
Attendees will learn how Kaiser Permanente’s Care Management Institute identifies health care delivery models that improve care for these patients through holistic treatment leveraging the resources of its integrated delivery system. Attendees will also learn about the National Center for Complex Health and Social Needs Blueprint for Complex Care, which provides a common framework for collectively advancing this emerging field. Key takeaways will include the:

  • Importance of cross-sector collaboration and data sharing to bring about new ideas and approaches.
  • Need for new healthcare payment models that integrate social services.
