The Science of Model Building for Complex Health and Social Needs was the first plenary session of December 2016’s Putting Care at the Center conference, the inaugural conference of the National Center for Complex Health and Social Needs.

Historically, innovative care models have been created in response to deficiencies in the system. What can we learn from successful existing models to inform the future of complex care? This plenary session featured pioneers and stewards of evidence-based care models for subpopulations with complex health and social needs. Their nuts and bolts discussion focuses on the what, why, and how of building, scaling, and sustaining successful models of care.
